The T2-FLAIR mismatch sign on MR imaging, usually determined by visual assessment, is a highly specific imaging biomarker of IDH-mutant astrocytomas. The authors of this study quantified the degree of T2-FLAIR mismatch in nonenhancing diffuse gliomas. Thresholds of ≥42% T2-FLAIR mismatch volume classified IDH-mutant astrocytoma with a specificity/sensitivity of 100%/19.6% (TCIA) and 100%/31.6% (institutional). They also found that grades 3–4 compared with grade 2 IDH-mutant astrocytomas (P<.05) had a higher percentage T2-FLAIR mismatch volume.

Temporal Characteristics of CSF-Venous Fistulas on Dynamic Decubitus CT Myelography: A Retrospective Multi-Institution Cohort Study
Andrew L. Callen, Mo Fakhri, Vincent M. Timpone, Ashesh A. Thaker, William P. Dillon and Vinil N. Shah
    American Journal of Neuroradiology January 2024, 45 (1) 100-104; DOI: https://doi.org/10.3174/ajnr.A8078

This retrospective multi-institution cohort study analyzed the temporal features of CSF-venous fistula (CVF) visualization on dynamic decubitus CT myelography (dCTM) in 48 patients. The results showed that most CVFs were visible on first or subsequent phases of dCTM, but approximately 1 in 8 were only visible on either the early or delayed phase. The authors conclude that acquiring greater than 1 phase of imaging increases the sensitivity of dCTM by increasing its temporal resolution.

    American Journal of Neuroradiology January 2024, 45 (1) 1-8; DOI: https://doi.org/10.3174/ajnr.A8038

Transcranial MR imaging-guided focused ultrasound enables precise delivery of energy through an intact skull for the treatment of various neurologic disorders. Real-time and noninvasive MR thermometry is critical to achieve precise and controlled transcranial focused ultrasound treatments. This article provides an updated review on clinically applicable considerations of proton resonance frequency MR thermometry, including pitfalls and limitations, to avoid complications during MRgFUS procedures.
